We want to make sure our media permissions are up to date for everyone. If you do not complete the form by Wednesday 11th November we will assume that you give permission for us to use your child's photo in the scenarios detailed but it is our preference that as many people as possible fill out the form. Please complete one form for each of your children. Thanks for your help.
